Here's my worst IT tale:

Two jobs ago, I was on a process and software re-engineering project which hired a major Indian consulting firm. Even though their Project Manager was excellent, and they were CMM level 4 certified, they provided some of the WORST documentation, and the project cost balooned from $13M to $30M over the two years.

Not only that, in the time it took us to develop requirements, an off the shelf product was released that would have given us what we designed. So we never built the software.

On my next job, the SAME consulting firm was being used, with similar results. They provided little or no requirements. However, they did deliver some of the worst looking code we have ever seen - it looked like 50 jr. developers developed their modules in isolation, and then just threw it all together.

Poor requirements management, as well as lack of hard deadlines and technical reviews along the way were our part of the fiasco. We don't do that anymore ;)

At the time I was a Server Admin that managed Novell and Windows servers. It was 2008 and we were still running an NT4 domain… yeah I know I should win just for that being the worst story ever, actually we still have one NT4 Domain still in existence, seriously… but I don't manage those servers anymore. So anyway, back to the story.

Our PDC for our NT4 Domain crashed and when we opened up the case and examined the hard drive we found the following sticker on the hard drive, look toward the bottom... http://twitpic.com/shiqh

Not to mention that it was a workstation class machine… oh the joys of working for a university…
